How to use our services
There are several pathways for people to be eligible and access SPDL services based on your age.
Please find the relevant information below.
My Aged Care
Eligibility requirements:
You are 65 years of age or over.
You must be registered with My Aged Care and be
on a Commonwealth Home Support Package (CHSP) or Home Care Package (HCP).Get a referral to SPDL via My Aged Care (1800 200 422)
Home and Community Care Program for Younger People (HACC PYP)
Eligibility requirements:
You must be under 65 years of age.
You can self-refer to us or via a Care Provider/The City of Port Phillip
National Disability Insurance Scheme (NDIS)
Eligibility requirements:
You must be under 65 years of age.
Get referred through yourself (if you manage your NDIS package) or your NDIS Provider.
We offer support to people who,
are National Disability Insurance Scheme (NDIS) Clients.
are frail, have chronic ill-health, injured or the ageing process have limited opportunities for social connections.
have a disability and not registered for and receiving services through the NDIS.
are unable to access mainstream public transport because of geographical isolation.
are living at home, in rental accommodation, living with family, in a boarding house or special accommodation house.
have a temporary medical condition and who may require service for a short period of time.are in need of short-term assistance due to an unexpected injury or other related health concerns (This is managed on a case by case basis).
use wheelie walkers who can access the DayLinks service.
Unfortunately, currently we are unable to support people who are in a wheelchair or are under the age of 18.
